Now look at what has not happened to the shoulders of pitchers Brad Penny and Jonathan Papelbon [stats], each with an injury history, and note the imminent activation of John Smoltz a year removed from shoulder surgery. This is what is called a teachable moment. Matsuzaka is on the disabled list for the second time in 2009 with shoulder weakness. Nobody yet can guess about if or when he will return this season. The Red Sox [team stats]’ deep and effective pitching staff is healthy now, and everybody has been on the team’s shoulder and arm strength and conditioning program since last year ended. Except Matsuzaka. The 28-year-old stayed in Japan all winter and in the spring. While his teammates gathered in Fort Myers for spring training, Matsuzaka stayed overseas to train with his Japanese teammates for the World Baseball Classic. Japan won the WBC for the second time in as many tries with Matsuzaka throwing 14 MVP innings in four starts, the fourth-most innings pitched of any tournament pitcher. Matsuzaka did not come under direct Red Sox supervision until March 27. The team already said that Matsuzaka’s involvement in the WBC is behind his two trips to the DL because he was throwing pitches in earnest before his body was ready. Matsuzaka was not ready because he was not on the Red Sox’ shoulder program. Making it clear that he was not speaking about Matsuzaka or the differences between Japanese and American throwing philosophies as they relate to shoulder preservation, Red Sox medical director Tom Gill spoke Sunday about the program. “The program itself allows you to lay down a foundation,” Gill said. “The baseball season is so long. To draw an analogy, the baseball season is the house and the offseason is the foundation. What you see as a pitcher goes deeper into the season is a breakdown. In a situation where players don’t work out as much as they should in the offseason, you can’t use the season itself to strengthen the shoulder. You do not strengthen the shoulder by throwing from the distance of the mound to the plate. You actually de-condition as you pitch like that. “When you are in spring training and you begin to do more and more long-tossing, that’s when you build strength in the muscles around the scapula, not by pitching. That’s why pitchers long-toss during the season as well, because in the act of pitching, you lose strength in your shoulder.” It is close to a daily miracle that more pitchers do not get hurt, and that pitchers’ careers last as long as they do. “We want to make sure that guys are not doing anything that can weaken the shoulder. The emphasis is on preventing injury,” said Gill, who described a “seamless system” of communication and trust between him, pitching coach John Farrell and pitchers rehab coordinator and assistant trainer Mike Reinold. Penny and Papelbon rave about the program. Not until Matsuzaka made his last start Friday did he state it was time to try a different approach. It could be that Matsuzaka has finally come around to accepting that if he is going to pitch every five days, instead of the once a week he did in Japan, there is value to the strength and conditioning program and not sticking to what worked for him in Japan. “You have to work closely with the pitching coach and everyone and build a level of trust high enough so they can say, ‘I’m tired’ or ‘I might have to miss a start or miss an appearance,’ ” Gill said. “Communication and a level of trust, we want them to believe in this system. “Biomechanics are biomechanics, and we think we have a pretty good grip on that. I think we’re part of a minority of teams that focus on offseason programs as much as we do. We want to try to control as many factors related to throwing and conditioning and strengthening as we can.” The Red Sox [team stats] lost control of Matsuzaka in the offseason and spring training.
